| Two employees were moving two pin protectors to another location to weigh them. They were trying to stand the two pin protectors up on end to secure them to a forklift with a chain and bolt. As they stood the pin protectors up, they attempted to pull slack out of the chain. One employee accidentally bumped heads with the injured employee, which caused him to stumble and lose his grip as he put his hand on the ground to balance himself.
